There is a $40.75 Freedom of Information (FOI) application fee that must be paid to the agency that holds the documents. Once the application is submitted, please contact Customer Service on 8555 7000 to process the fee for your FOI Request to be actioned.

If you are seeking to have the application fee waived, please upload evidence below supporting why it should be waived e.g. upload a copy of your concession card or other evidence as proof of financial hardship.
If no fee has been received and you do not qualify for a waiver, then this application will not be valid until the fee has been received.
